Unfortunately, all the Manhattan spas are as unfriendly to nudity as ever. Juvenex is probably the worst (I actually saw someone asked to leave once, and not in a polite way either), but I wouldn't feel comfortable going nude at any of them. You can get away with it at Spa 88 if there's no one else in the room, but hell, you can get away with it anywhere if there's no one else in the room...
I've heard good things about Dolphin, but I've never been there.
