It's not where, it's who. If you visit the same spa enough times, see the same masseuse each time, are polite and friendly every time, and then, after maybe three or four visits, ask very politely, you can generally get a hand release.
I remember the old Stressless Step, which was as professional and legitimate as a spa could get. They even had a posted policy forbidding you from tipping the staff at all, for anything, which I assume was in part to eliminate any risk of improper "extras" being requested. And there were some masseuses on staff you'd clearly never ask, because they very clearly would throw you out if you did (you can tell). But there were also some who were younger and friendlier and more casual, and after you'd seen one a couple of times you could start by saying, "Listen, do you mind if we leave the towel off this time? I'm just more comfortable naked." And sometimes they'd say no, but sometimes they'd be willing. Then maybe when they're working on your legs and you start to get hard, you wait for them to notice and say, in a sort of embarrassed way, "Sorry about that. I'll have to take care of that when I get home." A gentle hint, which they can pick up on or not.
Eventually, in more cases than not, you can find SOMEONE at every spa who is willing to do it.
